Els artistes pop Peter Blake i Jann Haworth van dissenyar la portada de Sgt. Pepper, guanyadora d'un premi Grammy a Millor Portada, a partir d'un esborrany de tinta fet per McCartney. La portada de l'àlbum consistia en un collage colorit protagonitzat pels Beatles vestits com a membres de la banda de Sgt. Pepper, acompanyats per un seguit de persones famoses.
El collage inclou 57 fotografies i 9 estàtues de cera que representen diverses personalitats, entre les quals actors, esportistes, científics. La imatge definitiva incloïa cantants com Bob Dylan i Bobby Breen; les estrelles de cinema Marlon Brando, Tony Curtis, Marlene Dietrich i Marilyn Monroe; l'artista Aubrey Beardsley; el boxador Sonny Liston i el futbolista Albert Stubbins. També hi figuraven els comediants Stan Laurel i Oliver Hardy i els escriptors H. G. Wells, Oscar Wilde, Lewis Carroll i Dylan Thomas.
